Glens Falls Hospital recognized as an Age-Friendly Health System

GLENS FALLS | Glens Falls Hospital has earned special recognition as an Age-Friendly Health System – Committed to Care Excellence hospital. This recognition was awarded by Agre Friendly Health Systems – an initiative of the John A. Hartford Foundation and the Institute for Healthcare Improvement in partnership with the American Hospital Association and the Catholic Health Association of the United States The hospital collected data on the number of older adults who received a proven set of high-quality care practices known as the 4Ms: What Matters, Medication, Mentation, and Mobility. More than 5,200 hospitals, medical practices, convenient care clinics, nursing homes, and home health organizations have received this recognition, and now Glens Falls Hospital proudly joins them. Worldwide, more than 5.47 million older adults have received care based on the 4Ms framework. "Our commitment to sharing data and learning continues to grow as we improve care for older adults," said Hillary Alycon, DrPH, MPH, FACHE, Associate Vice President for Quality Management and Infection Prevention. "We are working to consistently provide age-friendly care in our emergency department, intensive care unit, medical-surgical unit, and ultimately in our primary and specialty care practices."
